On examination, patient is BAR and hydrated but has a frequent dry cough, followed by the occasional hack. Left hock and lymph node remain small and comfortable on palpation. Discussed with foster continued concern that patient has pneumonia and may be developing reflux from constant hacking, as well as likely GI upset due to multiple antibiotics. Performed 4DX- negative x 4. Gave 400 mL SQ fluids, cerenia and famotidine injection. Dispensed additional week of AmoxiClav, Baytril, and Fortiflora. Dispensed Omeprazole (40 mg) PO BID x 7 days and two boxes of Cerenia (24 mg). Gave bag of Fresh Pet. Recommend discontinuing doxycycline at this time- if hock begins swelling again, may need to restart.(2/7) Patient presented to clinic for recheck of swelling in left hind hock- swelling has nearly completely resolved and popliteal lymph node has returned to normal size. Foster reports patient has developed exercise intolerance and still has a cough and occasional elevated respiratory rate despite course of doxycycline. Otherwise, patient is doing well- eating, drinking, urinatingand defecating normally. Blood was drawn and urine collected for CBC/Chem/UA/TT4. Consulted with IDEXX veterinarian. Discussed concern for possible low grade pneumonia and tick borne illness responding to doxycycline. Recommend complete course of enrofloxacin, extending AmoxiClav and completing 30 day course of doxycycline. Then, repeat blood work to assess thyroid. Neuter once URI/pneumonia has resolved for suspected BPH with possible prostatitis.(1/21) Vet Check - Patient is a senior, over conditioned dog with a large swelling around the left hock with pressure sores and a swollen local lymph node- r/o soft tissue infection vs septic arthritis vs injury vs neoplasia. Recommend starting with senior blood work and radiographs. Stable but urgent. Requires promotion for rescue willing to provide needed diagnostics and care. Behavior Observation: (1/22) Lifesaving - Stationary lying in front of kennel, allows leashing easily, halts going back into kennel, cannot be lured with treats, whale eyeing. Had to be muzzled and carried/lifted back into kennel. Minimal pulling on leash. Slow walker, lots of sniffing. Tense/allows some handling. Flinches to touch. Takes treats gently. Sniffs assessors but does not solicit interaction. TTA. (1/21) Vet Staff - Quiet, friendly. (1/20) Finder - Nice, friendly, calm, takes treats gently.This animal is currently available from foster care through ACCT Philly's Adoptions Programs.
